Information about autism diagnostic assessment, the professionals involved and Brazilian legislation.
What is diagnostic assessment?
Autism diagnostic assessment considers developmental history, clinical observation, communication, behavior, everyday functioning and information from people who know the person. Standardized tools may support assessment but do not replace clinical judgment. No single blood test, imaging exam or questionnaire can independently confirm or rule out autism.
The number of visits and professionals involved varies according to age, context, needs and service organization. In Brazil’s public health system, primary care units are an entry point and may refer to specialized services when needed.

Who can participate in the assessment?
Depending on the person’s needs, assessment may involve professionals from different fields, respecting each profession’s legal scope:
- Primary care physicians or specialists, depending on the case.
- Psychologists, when psychological, cognitive or behavioral assessment is needed within their scope.
- Speech-language pathologists, for communication, language, speech and feeding aspects within their scope.
- Occupational therapists, for participation, occupational performance, daily activities and function-related sensory aspects.
- Other professionals may contribute according to identified needs.
Who can issue reports or diagnostic documents?
There is no single document that serves every purpose in the same way. Reports and professional opinions must be issued within the responsible professional’s scope, and the authority granting a specific right may define which document it accepts.
When a rule requires a medical report
CIPTEA, for example, is issued upon application accompanied by a medical report containing the ICD code and other information required by Brazilian law. This does not mean every autism-related right requires the same document.
Multiprofessional reports
Professionals from different fields may document findings and needs within their scope. These records can support assessment and planning, but they should not be presented as if they all had the same legal purpose or professional authority.
For specific rights, check the applicable rule and responsible authority. Under current federal inclusive education rules in Brazil, for example, school support needs are assessed through a case study and do not depend on a health report.
Concern, assessment and diagnosis
1. Concern
Parents, caregivers or professionals notice signs that raise concerns. A concern is not a diagnosis. It is a reason to seek qualified professional guidance.
2. Assessment
A clinical and developmental assessment process. It may involve different professionals, screening or assessment tools, observation and interviews as needed. Tools should not be interpreted in isolation as a diagnosis.
3. Diagnosis
A clinical conclusion based on the relevant information as a whole. Documentation depends on the professional, service and purpose of the document. Communication should respect the person and family and avoid reducing identity to a diagnosis.
The importance of individualized assessment
Individualized assessment can help identify needs, strengths, associated conditions and everyday barriers. A diagnosis may facilitate access to care and rights, but it does not by itself determine appropriate supports or predict a person’s future path.
