ABA in the SUS: what is right, what depends on the network and what should not be promised

A guide for families to understand the care for autistic people in the SUS without confusing the right to comprehensive care with the automatic guarantee of a specific method, professional or workload.

What the law guarantees

Law No. 12,764/2012 provides comprehensive care for the health needs of autistic people, including diagnosis, multidisciplinary care and access to medicines and nutrients. The LBI reinforces habilitation and rehabilitation rights when necessary.

How care is organized

The Ministry of Health guides a Unique Therapeutic Project, built based on the needs of the person and the family, with shared decision-making and review over time.

What varies locally

Teams, services, professionals, queues, frequency and available modalities may vary depending on the municipality, region and installed capacity of the network.

ABA is an approach. The right in the SUS is to comprehensive and individualized care.

Applied Behavior Analysis brings together principles and procedures derived from behavioral science that can be used for specific objectives. This does not mean that every autistic person needs the same program, that ABA replaces all other areas, or that a standardized course load is suitable for everyone.

The Ministry of Health's current page on the Singular Therapeutic Project records that there are different interventions for ASD and states that there is no scientific evidence of the superiority of a single approach over all others. Therefore, the focus must remain on needs, functional objectives, quality of life, participation and reevaluation of care.

Questions that most confuse families

Frequently asked questions about ABA and SUS

Is the SUS obliged to offer ABA to every autistic person?+

Federal legislation guarantees comprehensive health care, multidisciplinary care and habilitation and rehabilitation services when necessary. In the federal sources consulted, there is no national rule that imposes a single therapeutic approach, a specific ABA protocol or a fixed number of hours for every autistic person. The Ministry of Health recommends that care be individualized and organized according to needs and objectives.

So ABA cannot exist in the SUS?+

There may be professionals and services that use principles, strategies or behavioral interventions, depending on the team's formation, the person's needs and the organization of the local network. This is different from stating that the national SUS guarantees a standardized ABA package in all municipalities.

Where should the family start?+

Primary Care, normally through UBS, is the preferred entry point. Depending on the need, there may be referrals to CER, CAPS/CAPSi, polyclinics, multidisciplinary teams or other reference services in the local network.

Do I need to arrive with a treatment already chosen?+

No. The family can bring questions, previous assessments, reports and daily priorities. The care plan must consider shared needs, potential, preferences, context and objectives, rather than just starting with the name of a technique.

What can I ask at UBS or the specialized service?+

Ask what the municipal flow for ASD is, which professionals make up the team, how the Singular Therapeutic Project is prepared, what objectives are being monitored, how the family participates in decisions and how the plan will be reassessed.

What if the service I need doesn't exist in my region?+

Availability varies between municipalities. The reference team must guide the existing flow and possible referrals. For administrative information, regulation, ombudsman or municipal network, always confirm the official channels of your city and SUS.
